One You Lincolnshire and Thrive Tribe are delighted to partner with K2 Healthcare Sleaford, South Lincoln Healthcare, First Coastal, First Coastal Rural Division and Trent Care Network PCNs to employ 11 Health and Wellbeing Coaches as part of the Additional Roles Reimbursement Scheme (ARRS).
The new staff have already begun working with the It's All About People Personalisation Programme Team to explore how they will use tools such as the Patient Activiation Measure and Personalised Care and Support Plans.
Specialising in behaviour change, the Health and Wellbeing Coaches will deliver face-to-face clinics, and tele-coaching sessions, to provide extra support for people who want to make lifestyle changes that support their long-term health and wellbeing.
Concentrating on what really matters to each person, we will support people to explore their health issues and work in collaboration with them to focus on habits and behaviours, motivation and confidence, barriers to change, and long-term health condition management.
These sessions allow people to reflect on current behaviours and assess what changes are essential to them, what goals are realistic for them and what they can focus on as they start to move towards happier and healthier lives.
Delivery of the sessions will start in November. Referrals to the Health Coaches will come from the Primary Care Network teams.
