Through our regular conversations with colleagues from Primary Care Networks, we discovered that their clinical system templates are not fit for purpose and don't meet the PCSP quality assurance criteria - they therefore have to work around the systems and processes to be able to record what matters and what's important to people.
A recurring frustration for people is having to repeat their story to a range of health and care professionals - stories that can be trauma-inducing and shouldn’t need to be repeated.
We therefore set up and hosted a Personalised Care and Support Planning (PCSP) workshop for partners from nine Primary Care Networks at the New Life Conference Centre in Sleaford on 31 January 2024.

From 1 February, the information standard for PCSP becomes mandated across NHS organisations, to ensure that there is consistency in where, how, and what is recorded.
We now have an Information Standard Working Group that has the responsibility for ensuring Lincolnshire’s NHS organisations are compliant. The group has agreed that to meet the standard, consistent adoption of the Lincolnshire Care Record is required. This includes the introduction of the ‘in context’ switch in System One, which will take colleagues straight to the Care portal without a separate login.
The funding for this work has been approved and Primary Care are top of the list.
During this workshop we looked at what needs to be consistently recorded in a PCSP.
Anthony Gorman from the NHS Lincolnshire ICB digital team updated the workshop about the upgrade of the Care Portal and the Shared Care record and helped to bust some myths about recording what matters to me plans (personalised care and support plans) directly onto the care portal.

The session about the Quality Assurance framework, enabled colleagues to start thinking about their level of maturity in PCSP and what actions they needed to take when they were back in their wider teams.
