Mapping impacts at the Personalisation Champions March catch-up

This is just some of the wonderful feedback we received following the It’s All About People Personalisation Champions Spring event, which took place on Wednesday 18 March at Branston Village Hall. 

Attending this session were Champions from NHS organisationsLincolnshire County Council, several different voluntary and community organisations, as well as people with lived experience

As with all of our Champions events and interactions, social time is a key element - time to catch up with fellow Champions from across the Lincolnshire health and care system, share stories and news, and, yes, have a laugh - PLUS network and make valuable new contacts, of course!

Coming together regularly in this way ensures we all are aware of what is going on across different services, and different parts of the county. And there's no better example of the Our Shared Agreement Foundation Three, Working together for the wellbeing of everyone than a Champions get together!


Mapping personalisation impacts

Many great relationships have been developed via our Champions network, and these have in turn led to different and better ways of working when providing support to people. 

So, at this session, we asked the Champions to share the impact ‘ripples’ their work was causing, and explored how these ripples might be converted into ‘waves’ of change across the system. 

To see where personalisation approaches are making a difference across Lincolnshire, we used the Padlet mapping app (see the image below) and invited our Champions to record their interventions and impacts on the map.

Behind every pink marker, are stories of personalised care and strength-based approaches in action, and the impacts of this essential work.  

Here are a few examples:

  • Skegness - Supported the Primary Care Network (PCN) to share their story of using Population Health Management (PHM) and personalised approaches on the national stage at NHS Confed Expo 2024. A team member from the PCN joined our panel and we started the session with a person's story, demonstrating the power of personalised care and PHM combined to drive better outcomes.
  • Gainsborough - Colleagues raised concerns about our ability to support people with their pets when they need admission and won't go to hospital. Their pets are important to them and they are concerned what will happen to them while they are in hospital. A shout out to colleagues is helping us build a better understanding of the support available across the county.
  • Spalding - Attended Shared Decision Making (SDM) and Personalised Care and Support Planning (PCSP) training, which has all been reflected in my clinical work - changing the way assessments are completed and acknowledging that changes were needed to ensure that each assessment is individualised for that patient.

This mapping process also shows us where the gaps are and where, as a team, we could focus attention to influence real change. 

Please note - this map only records the personalisation stories of those Champions who attended this particular meeting. Across Lincolnshire, there are many other Champions, along with health and care colleagues who aren't Champions, working to embed and promote personalised and strengths-based approaches. Adding their stories to the map will continue to fill the gaps we see above.  

It's our aim to cover the whole of Lincolnshire with pink markers...and you can help!


Help us to evidence the impacts and outcomes of personalisation

We are going to continue to use this map as an active 'Personalisation in Action' recording device.

We want anyone who attends a personalisation event, completes any of our Learning and Development training, or seeks support from the It’s All About People team, to log the impact this has had on them, their service, the system, and, of course, the people they support. 

We know personalisation is making a huge impact on health and care provision across the county. We know personalised care and strengths-based approaches are building a better relationship between the public and the services that support them.

So, we are gathering as much feedback and evidence of impact as we can, both from our colleagues in the health and care system, and from the public we all serve of course.
