Personalised Care and Support Planning in Primary Care

On Wednesday 20 September 2023, colleagues from Primary Care Networks (PCNs) across Lincolnshire convened to discuss Personalised Care and Support Planning (PCSP). 

Participants shared information about statutory requirements, best practice, and the challenges faces by the system.

The session was incredibly valuable and participants all agreed that they would like to come together again to support each other.
