Introduction to Personalised Care and Support Planning

Personalised Care and Support Planning (PCSP) is an umbrella term that covers the planning approaches that teams, services and organisations use to have ‘What matters to you?' conversations with people.

‘What matters to you’ conversations are recorded in a Personalised Care and Support Plan (also abbreviated to PCSP) and should be seen as the first chapter in someone's ‘book’.

People with a number of long-term conditions often have a range of treatment and care plans that are additional chapters in their ‘book’ and should be viewed alongside their personalised care and support plan.

Personalised Care and Support Planning training

The It’s All About People team offer three different levels of training to provide more information about Personalised Care and Support Planning. Each course has a different level of complexity and is suitable for those with particular job roles or based on the interaction they have with people.

All of our Personalised Care & Support Planning is accredited by the Personalised Care Institute. You can find us listed on the PCI website.

Level 1: Personalised Care and Support Planning Introduction (e-learning module)

Course Overview: Getting Personalised Care and Support Planning (PCSP) right is essential for people to gain more choice and control over their lives and the support they are receiving. This 45-minute e-learning module will provide you with a definition and overview of the criteria for a good PCSP and highlight - through a range of case studies - the key steps of the planning process. 

Who is the training aimed at?: This training is for all staff in health, social care, or the third / voluntary sector who work in Lincolnshire.

Level 2: Personalised Care and Support Planning (virtual workshop)

Workshop provider: It's All About People

Workshop overview: Different types of treatment and care plans are often put in place for people across services in Lincolnshire. People can end up having multiple plans depending on how many long-term conditions they have. The plans don’t often capture what’s important to the person and don’t always make sense to them.

This workshop is delivered by Lincolnshire practitioners who understand the local issues and pressures you may face, and is an interactive session over Teams which enables you to have open conversations with colleagues.

Level 3: Personalisation in Care & Support Planning (Advanced)

Course Overview: Every-One is committed to the promotion and facilitation of personalisation and has delivered training and engagement across Lincolnshire in a range of settings.

Every-One's core mission is ‘Making Wellbeing Personal’ and they have become a flag-bearer for person-centred approaches in Lincolnshire and beyond whilst being a resource for supporting others to embed the personalisation agenda as business as usual.

Every-One sees personalisation as a fundamental way for people to have choice and control over their wellbeing, and to influence the design and delivery of better services.

This training aims to ensure that those involved are on the same page with regards to personalisation and Personalised Care & Support Planning, and have opportunities to reflect on their practice.

The training offers a bespoke programme to meet the needs of the target cohort by supporting staff to have the skills, knowledge, and confidence to apply person-centred approaches to care and support planning. This course includes:

  1. The national and local context for Universal Personalised Care
  2. How to incorporate your local models of personalisation
  3. How to introduce personalisation in care and support planning, and the tools you can use.
  4. How to use reflective practice to embed positive behaviours to support personalisation
  5. A series of case studies as examples of learning

The reflective sessions are interactive and attendees are expected to come prepared to discuss, debate, and share their experiences of Personalised Care and Support Planning based on exercises set for them at the previous input session.  

At the end of the programme attendees are expected to complete their own short case studies and their own One-Page Plan – templates for these are included in this pack. 

How will the training be run?: The sessions are run over a few months. Reflective practice is built into the programme, so participants are expected to try things out as they go along and then share as a group what they have discovered in their practice.

Sessions can be delivered online using Teams or face-to-face.

Who is the training aimed at?: This training is for people / patient / client facing staff in the health or third / voluntary sector who work in Lincolnshire and want to be able to regularly use Personalised Care and Support Plans and / or lead a team to do so.

Time to complete:

The programme is based on 14-hours contact time and is usually made up of:

  • 4 x 2 hours Input Sessions
  • 4 x 1 ½ hours Reflective Sessions

The requirement to gain accreditation is to attend all 4 sessions.
