Leadership (Team)

The Lincolnshire Neighbourhood Team Leads (Clare Credland, Louise Johnson, Ruth Taylor, Matthew Fisher and Nathalie Munslow) are awarded in recognition of their outstanding leadership in personalised care within their Neighbourhoods. They have inspired and challenged all of the teams that they work with to always think about the person and what's right for them as opposed to what the process should be. 

They have supported the co-production and embedding of What Matter's to Me conversations and PCSP, Care Co-ordination model, promoted personalised care training within the neighbourhood teams and wider primary care teams, championed the voice of the person/carers/families in any system project/programme they have been involved in and much, much more. 

They continue to be an asset to our county. Well done!
