Outstanding Contribution (Team)

The Community Neuro-Outreach Team strive to ensure each individual person is offered the best support and care based on the needs and wishes they identify, with the Team working hard to support improved communication between services to ensure people are fully supported. They work tirelessly to establish and maintain strong links across the County, with specialist centres and the MNDA in order to make the most of what is available. They are a pro-active, multi-disciplinary, specialist team, ensuring people feel valued and empowered to make their own decisions regarding their care during a time when they often feel helpless and out of control. They are supportive of the person and also the family and maintain an individualised, personalised approach; focusing on what is important to the individual while helping them manage the devastating diagnosis/prognosis.
